The 340B Rebate Model Pilot goes live on January 1, 2027, and our team is ready with Rebate Intelligence. The solution fully automates rebate submission and reconciliation workflow, reducing the manual burden on pharmacy teams and protecting 340B savings when rebate payments become incomplete, delayed, or denied.

With Rebate Intelligence, covered entities gain:

  • Rebate reconciliation workflow automation — End-to-end automation for the 340B Rebate Model Pilot, from identifying 340B rebate-eligible claims to submitting complete, validated data sets through Beacon.
  • Daily submission management — Plenful maintains a consistent cadence of daily submissions to help prevent delays or disqualifications. Claims missing required data elements are flagged in a work queue for resolution before submission.
  • Rebate executive dashboards and insights — Visibility into centralized cash flow across multiple TPAs and clean sites, and claim-level insights across the enterprise, at macro or covered entity-specific views.
Plenful's Rebate Intelligence automates the entire rebate lifecycle, from identifying eligible claims to reconciling payments.

30+ health systems are preparing with Plenful

Today, more than 30 leading covered entities have signed on, signaling trust in Plenful to navigate this enormous regulatory complexity.

"The 340B Rebate model still has a lot of unanswered questions, and this is a major operational shift on a tight timeline," said Christopher Hannon, Director, 340B Program at Northwell Health, New York State's largest health system. "Manual tracking of rebates simply can't hold up to the volume and data integrity the program demands. Plenful has earned our trust with the impact they delivered for our 340B compliance audit initiative, and their rebate product will streamline every step of the process. It doesn't get easier than that, and it means our team can stay focused on patients and compliance with the 340B Program in this new reality."

The Rebate Model puts $2.75 billion in 340B savings at stake

The 340B Drug Pricing Program helps nearly half of U.S. hospitals stretch scarce resources to care for underserved patients. Under HRSA's revised Rebate Model, a subset of those savings will reach covered entities differently. Instead of an upfront discount, covered entities will pay wholesale acquisition cost (WAC) on select drugs within Medicare's Drug Price Negotiation Program for 2026 and 2027, then submit rebate claims and wait to be repaid the difference.

Covered entities purchased an estimated $100 billion in 340B drugs in 2025. The drugs covered by the Rebate Model make up about 5.5% of that volume, or roughly $5.5 billion in annual 340B purchases. At a typical 340B discount of around 50%, an estimated $2.75 billion in 340B savings is at stake.

If a rebate submission is incomplete, delayed, or denied, your 340B savings are at risk. 340B Health previously warned that a rebate model would require safety-net hospitals to float millions of dollars annually while awaiting reimbursement, with the burden scaling sharply for larger hospitals.

Daily claim-level submission is expected to become the operational standard to keep that risk as small as possible. That cadence has no precedent under the traditional 340B discount model, and manual, spreadsheet-based tracking will be difficult to sustain, particularly for teams that are already stretched thin.

Plenful offers one command center for Rebate and MFP

Rebate Intelligence expands our Claim Reconciliation Management suite alongside MFP Intelligence, our solution for reconciling Medicare Maximum Fair Price refunds. MFP Intelligence brings together MTF data, 340B eligibility data from pharmacies, and financial workflow into a single platform. Instead of chasing down discrepancies, your team can proactively identify issues, monitor key metrics, and ensure every dollar is accounted for.

Our GFI Agent automates refund recovery, fully automating Good Faith Inquiry submissions as well as managing and completing the manufacturer requests connected to each GFI, such as providing specific invoices and associated claims to Beacon. Customers using MFP Intelligence have identified as much as 20% of expected MFP refunds as missing.

Together, the two solutions give your pharmacy and finance teams a centralized command center across both programs.
